Look up an Oklahoma plate
See whether other drivers have filed reports against a plate.
A plate with no reports comes back clean — that is the common result, and it is not an error.
Avoidance & safety rules
- Do not follow or pursue the vehicle, under any circumstances.
- Do not try to read or photograph the plate if doing so means getting closer or driving unsafely.
- Do not engage. No gestures, no eye contact, no reply to an aggressive driver.
- Keep your distance and let them pass. Pull over somewhere public before you make a call, or have a passenger call.
- Note details only if it is safe: make, model and colour, full or partial plate, direction of travel, and the exact location or mile marker.
Does a Jerk Alert report reach the Oklahoma Department of Public Safety? No — Jerk Alert filings are community records between drivers. They never reach the Oklahoma Department of Public Safety, any other government body, or law enforcement, and they do not replace the calls in sections 1 and 2. If you are in danger, call 911. The Terms prohibit following, approaching, or identifying another driver on any Jerk Alert platform.
Oklahoma bad driver reporting & safety guide
1. If it is happening now immediate danger
Emergency hotlines
- 911 — for a driver who is an active danger to other people right now. That is a police matter, and no app is a substitute for the call.
- No Oklahoma-specific line is listed here. We publish a patrol code only when a state police or DOT page carries it.
2. Reporting to Oklahoma law enforcement after the fact
Neither the hotlines above nor anything in this section is the licence-review route in section 3, and nothing filed on Jerk Alert reaches any of them.
Who takes the report in Oklahoma
- The non-emergency line of the Oklahoma municipal police department or county sheriff’s office with jurisdiction where it happened takes reports of dangerous driving that has already happened.
- Bring: the plate (full or partial) and state, the vehicle’s make, model and colour, the location, the date and time, the direction of travel, and what you saw.
- Expect a record rather than a ticket. An officer who did not witness the driving usually cannot cite for it, and what each agency acts on varies.
3. State DMV & licensing agency review
Public reporting accessYes — citizens can report dangerous drivers
A named form a member of the public can file with the licensing agency.
Who can file: Anyone with personal knowledge of the condition. The form's Source of Information boxes are Judge, Law Enforcement Officer, Licensed Health Care Professional, Private Citizen, Relative of Driver and Other.

- Questions about a request: (405) 425-2083 or (405) 425-2059, both printed on the form. The Department's Driving and Health FAQ gives Medical Desk (405) 425-2148 for a driver who cannot return a requested medical report on time.
What information do I need to report a driver?
- You must have personal knowledge of the condition — the Department's examples are a family member, doctor, judge or law enforcement officer.
- Every starred field must be completed, including the driver's licence number and mailing address, and your own name, title, signature, date and address.
- Describe in detail the circumstances that led to the request, your observations of the person's medical or visual condition, and the reasons you believe would prevent them from operating a motor vehicle safely. The form gives the behaviour indices the Medical Desk looks for: disorientation, mental confusion, senility, lapses of attention, behavioural changes, impaired reflexes, impaired balance recovery, orthostatic hypotension, or any condition which would cause loss of control or partial control of a motor vehicle.
- You choose what the driver should at minimum be required to complete: a medical examination, a vision examination, the driver licence written exam, or the driving skills exam.
- If a physician has diagnosed the condition, tick the box for it — amputation, visual loss, psychiatric, dementia, seizures, stroke, traumatic brain injury, Parkinson's, neurological, paralysis or multiple sclerosis.
- You certify that you are submitting the form in good faith without negligence or malicious intent.
- An emailed copy will not be accepted, because the Medical Desk requires the requestor's original signature to initiate a medical inquiry. Post the form to the address below and mark the envelope 'Attention Medical Desk'.
Will the driver find out who reported them?
The Department states that it cannot guarantee the person submitting the Request for Driver Review will remain anonymous throughout any review process started as a result of the request. That warning is printed on the form's own face and repeated in the Department's Driving and Health FAQ. Nothing the Department publishes says whether the driver is told who filed.
What can the Oklahoma Department of Public Safety do after a report?
- Signing and submitting the form permits the Department to conduct an investigation concerning the driver's driving and health issues.
- The review can require the driver to complete any of the four examinations the requestor chooses from on the form.
- The Department's own purpose rule, OAC 595:10-5-1, is to restrict, deny, cancel or disqualify the driving privilege of an applicant or licensee who presents an unwarranted risk or does not meet the minimum standards.
Statutory authority: 47 O.S. §§ 6-207 and 6-119, and OAC 595:10-5-13; the purpose rule is OAC 595:10-5-1.
What this route can and cannot do
A fitness-to-drive referral, not a bad-driving complaint: it is built around a medical, cognitive or physical condition and asks for the driver’s name and date of birth, which a plate number does not give you.
