South Carolina has no public reporting route.
A review of a driver’s licence can only be started by someone in a defined role — a physician, a law enforcement officer, a court. A member of the public cannot start one, however clearly they saw what happened.
If you are worried about a family member or someone in your community, the route runs through their doctor: their primary care physician must submit the medical concern to the SCDMV. A member of the public cannot submit it themselves, and cannot submit it on the physician's behalf.

An Erk Score is a plate’s running rating from 0.0 to 5.0, computed from the reports filed against it and weighted by whether the reporter is verified. Endorsements from other drivers are counted on the report itself and do not change the score. Deleted and hidden reports are excluded, so a score only reflects reports currently standing.
A plate with no reports has no score.
