Look up an Indiana plate

See whether other drivers have filed reports against a plate.

A plate with no reports comes back clean — that is the common result, and it is not an error.

Avoidance & safety rules

Does a Jerk Alert report reach the Indiana Bureau of Motor Vehicles? No — Jerk Alert filings are community records between drivers. They never reach the Indiana Bureau of Motor Vehicles, any other government body, or law enforcement, and they do not replace the calls in sections 1 and 2. If you are in danger, call 911. The Terms prohibit following, approaching, or identifying another driver on any Jerk Alert platform.

Indiana bad driver reporting & safety guide

1. If it is happening now immediate danger

Emergency hotlines

  • 911 — for a driver who is an active danger to other people right now. That is a police matter, and no app is a substitute for the call.
  • No Indiana-specific line is listed here. We publish a patrol code only when a state police or DOT page carries it.

2. Reporting to Indiana law enforcement after the fact

Neither the hotlines above nor anything in this section is the licence-review route in section 3, and nothing filed on Jerk Alert reaches any of them.

Who takes the report in Indiana

  • The non-emergency line of the Indiana municipal police department or county sheriff’s office with jurisdiction where it happened takes reports of dangerous driving that has already happened.
  • Bring: the plate (full or partial) and state, the vehicle’s make, model and colour, the location, the date and time, the direction of travel, and what you saw.
  • Expect a record rather than a ticket. An officer who did not witness the driving usually cannot cite for it, and what each agency acts on varies.

3. State DMV & licensing agency review

Public reporting accessYes — citizens can report dangerous drivers

A named form a member of the public can file with the licensing agency.

Who can file: Medical professionals, law enforcement, court officials, a power of attorney, family members, and any concerned citizen — the BMV's list.

What information do I need to report a driver?

  • A request from a concerned citizen or other entity must be filed on State Form 54750, Request for Driving Ability Review.
  • Identify the driver: name, telephone number, address including county, date of birth, driver's licence number and the date the licence expires.
  • Say why. The form gives you one large block under “I am requesting a driving ability review for the above named driver for the following reason”.
  • Tick your affiliation. The boxes are law enforcement (with badge number), prosecutor, BMV/C personnel, medical or general power of attorney, rehabilitation or medical centre, physician, court-appointed guardian or custodian — and Other, which is the box a concerned citizen uses.
  • Sign the oath. It reads: “I swear or affirm that the information I have entered on this form is correct. I understand that making a false statement may constitute the crime of perjury.” Print your name, date it, and give your address.
  • Give your full legal name, address, contact details and signature. The Driver Ability Department will not process any reviews from an anonymous source.
  • Complete it in blue or black ink and send it to the BMV at 100 North Senate Avenue, Room N481, Indianapolis IN 46204, or fax it to 317-974-1614. The form's header addresses it to the Driver Ability Department while its instructions say to mark it Attn: Medical Review Clerk — the BMV prints both on the same page, so either attention line is the agency's own.

Will the driver find out who reported them?

Not anonymous, and not protected against the driver. The BMV treats the file as confidential at intake, but Indiana law gives a reporter no absolute statutory confidentiality: if the driver challenges a restriction, suspension or revocation before the Office of Administrative Law Proceedings or on court review, they have the right to inspect their complete administrative file — which includes State Form 54750 and your identifying details.

What can the Indiana Bureau of Motor Vehicles do after a report?

  • The Driver Ability Department may, AT ITS DISCRETION, initiate an investigation to consider invalidating, revoking or restricting the driver's privileges.
  • On receipt the Department reviews the information and decides whether further investigation is necessary.

Statutory authority: IC 9-24-10-6 and IC 9-24-10-7.

What this route can and cannot do

Indiana is one of the few states whose process covers observed driving behaviour and not only medical impairment — but it is still a fitness-to-drive referral: it asks for the driver’s name and date of birth, which a plate number does not give you, and its remedy is a competence re-test that a capable but aggressive driver passes.
