Your local police department’s non-emergency line takes reports of dangerous driving that has already happened. What each agency will act on varies, and a report generally needs the plate, the location, the time, and what you saw.
There is no citizen form in Pennsylvania. The route is a letter you write yourself, sent to the unit below.
DL-13 (Initial Reporting Form) is a HEALTH CARE PROVIDER form, not a citizen form — PA law requires physicians and others authorised to diagnose or treat to report to PennDOT. That remains true, but it is NOT the whole picture: PennDOT also accepts a citizen letter, and says so on its own safety FAQ.
Among the strongest in the country. Under 75 Pa.C.S. § 1518(d) reports to PennDOT are confidential and exempt from public records requests, and PennDOT's medical policy states that it is compelled by law to honour that provision and will not release information about the source or content of the report even when the inquiry comes from the patient.
The law protects you for reporting. Under 75 Pa.C.S. § 1518(f), a person who reports in good faith has statutory immunity from civil and criminal liability.

This is a fitness-to-drive referral, not a bad-driving complaint. An Erk Score is a plate’s running rating from 0.0 to 5.0, computed from the reports filed against it and weighted by whether the reporter is verified. Endorsements from other drivers are counted on the report itself and do not change the score. Deleted and hidden reports are excluded, so a score only reflects reports currently standing.
A plate with no reports has no score.
