Hawaii has no public reporting route.
A review of a driver’s licence can only be started by someone in a defined role — a physician, a law enforcement officer, a court. A member of the public cannot start one, however clearly they saw what happened.
Hawaii's rules name the only sources of information the county examiner of drivers may act on: federal, state or county police authorities; licensed medical doctors and optometrists; federal or state courts; and the examiner of drivers or their authorized representatives (HAR 19-122-352). A member of the public is not on that list, and no county publishes a citizen reporting form.
Hawaii is one of 18 of 51 US jurisdictions where a witness unrelated to the driver has no confirmed way to report them to the licensing agency. That gap is the reason Jerk Alert exists.

An Erk Score is a plate’s running rating from 0.0 to 5.0, computed from the reports filed against it and weighted by whether the reporter is verified. Endorsements from other drivers are counted on the report itself and do not change the score. Deleted and hidden reports are excluded, so a score only reflects reports currently standing.
A plate with no reports has no score.
